Lincoln Self Reliance 

 Lincoln Self Reliance is a Wyoming Community Service Provider with offices in Afton, Thayne, and Diamondville.
LSR provides habilitation services to adults with developmental disabilities or acquired brain injuries and case management services for adults and children in their local communities.

Mission Statement:  
It is our mission to provide training and support to individuals with disabilities so they may achieve maximum potential for self reliance and active participation in the community.

Services Offered:
Some of the many services we offer to eligible individuals* in our communities are: 

• Day Habilitation Services & Community, Social, and Independent Living Activities
• Vocational services and community based/supported employment 
• Transportation to employment, appointments, and events
• Residential Habilitation (24-hour staffed homes) in three locations- Afton, Thayne, and Kemmerer
• Supported Living
• Skilled Nursing
• Case Management
• Ancillary Services
